GD is therefore expected to reflect predominantly the activity of type I deiodinase.

''GD'' is both a theoretical concept that is used in physiological theories of thyroid function and (as a calculated parameter) a biomarker for advanced diagnosis of thyroid disorders.<ref>{{cite book |last1=Hoermann |first1=Rudolf |last2=Dietrich |first2=Johannes W. |chapter=Diagnosis of Hypothyroidism |title=Hypothyroidism |date=2025 |pages=111–132 |doi=10.1007/978-981-96-3682-2_10|isbn=978-981-96-3681-5 }}</ref>

== How to determine G<sub>D</sub> == ''G<sub>D</sub>'' can be determined experimentally by exposing a cell culture system to saturating concentrations of T4 and measuring the T3 production. Whole body deiodination activity can be assessed by measuring production of radioactive iodine after loading the organism with marked thyroxine.<ref name="Bianco_2014">{{cite journal |last1=Bianco |first1=AC |last2=Anderson |first2=G |last3=Forrest |first3=D |last4=Galton |first4=VA |last5=Gereben |first5=B |last6=Kim |first6=BW |last7=Kopp |first7=PA |last8=Liao |first8=XH |last9=Obregon |first9=MJ |last10=Peeters |first10=RP |last11=Refetoff |first11=S |last12=Sharlin |first12=DS |last13=Simonides |first13=WS |last14=Weiss |first14=RE |last15=Williams |first15=GR |title=American Thyroid Association Guide to investigating thyroid hormone economy and action in rodent and cell models. |journal=Thyroid |date=January 2014 |volume=24 |issue=1 |pages=88–168 |doi=10.1089/thy.2013.0109 |pmc=3887458 |pmid=24001133}}</ref>

However, both approaches are faced with draw-backs. Measuring deiodination in cell culture delivers little, if any, information on total deiodination activity. Using marked thyroxine exposes the body to thyrotoxicosis and radioactivity. Additionally, it is not possible to differentiate step-up reactions resulting in T3 production from the step-down reaction catalyzed by type 3 deiodination, which mediates production of reverse T3. Distinguishing the contribution of distinct deiodinases is possible, however, by sequential approaches using deiodinase-specific blocking agents, but this approach is cumbersome and time-consuming.<ref name="Bianco_2014"/>

''In vivo'', it may therefore be beneficial to estimate ''GD'' from equilibrium levels of T4 and T3. It is obtained with

<math>\hat G_D = {{\beta _{31} (K_{M1} + [FT_4 ])(1 + K_{30} [TBG])[FT_3 ]} \over {\alpha _{31} [FT_4 ]}}</math>

or

The method is based on mathematical models of thyroid homeostasis.<ref name = "Dietrich_2016"/><ref name=dietrich2002/> Calculating deiodinase activity with one of these equations is an inverse problem. Therefore, certain conditions (e.g. stationarity) have to be fulfilled to deliver a reliable result.

The product of SPINA-GD times the urinary iodine excretion can be used to assess iodine-independent factors affecting deiodinase activity, e.g. selenium deficiency.<ref name="Sun_2023">{{cite journal |last1=Sun |first1=Q |last2=Oltra |first2=E |last3=Dijck-Brouwer |first3=DAJ |last4=Chillon |first4=TS |last5=Seemann |first5=P |last6=Asaad |first6=S |last7=Demircan |first7=K |last8=Espejo-Oltra |first8=JA |last9=Sánchez-Fito |first9=T |last10=Martín-Martínez |first10=E |last11=Minich |first11=WB |last12=Muskiet |first12=FAJ |last13=Schomburg |first13=L |title=Autoantibodies to selenoprotein P in chronic fatigue syndrome suggest selenium transport impairment and acquired resistance to thyroid hormone. |journal=Redox Biology |date=3 July 2023 |volume=65 |article-number=102796 |doi=10.1016/j.redox.2023.102796 |pmid=37423160|pmc=10338150 }}</ref>
