The river is {{convert|387|km|mi}} long and has a basin area of {{convert|11600|km2|mi2}}.<ref name=bse>[http://bse.sci-lib.com/article060073.html Кафирниган], [[Great Soviet Encyclopedia]]</ref> It rises on the southern slopes of [[Gissar Range]] in [[Vahdat district]], formerly Kofarnihon district, and flows in the general south-western direction past the cities of [[Vahdat]] and [[Dushanbe]], where it turns south and runs through [[Khatlon Province]] toward the [[Afghanistan–Tajikistan border|border with Afghanistan]]. It falls into Amudarya some 40 km west of the confluence point of Vakhsh and Panj rivers. For part of its course, it forms the [[Tajikistan–Uzbekistan border|border between Tajikistan and Uzbekistan]]. Its main tributaries are the [[Varzob (river)|Varzob]] and [[Khanaka (river)|Khanaka]] from the right, and the [[Elok (river)|Elok]] from the right.<ref name=bse/>
