It originates in the [Apennine Mountains](/source/Apennine_Mountains), in the [province of Isernia](/source/province_of_Isernia) and flows into the [Adriatic Sea](/source/Adriatic_Sea) near [Vasto](/source/Vasto). On 2 April 2026, following bad weather conditions, one of the bridges over the Trigno collapsed.
