It runs parallel to [Interstate 80](/source/Interstate_80_(Iowa)), which runs {{convert|10|mi}} to the south for most of Iowa 44's route. Iowa 44 begins at its junction with [U.S. Highway 30](/source/U.S._Highway_30) four miles (6 km) northeast of Logan. It ends at an interchange with the [Iowa Highway 141](/source/Iowa_Highway_141) freeway at Grimes. Iowa 44 was created in 1969 when [Iowa Highway 64](/source/Iowa_Highway_64) was shortened to its current route in eastern [Iowa](/source/Iowa). Most of the route is a part of the Western Skies Scenic Byway.

Iowa Highway 44 begins between Logan and [Woodbine](/source/Woodbine%2C_Iowa) on [U.S. Highway 30](/source/U.S._Highway_30). It goes east to [Portsmouth](/source/Portsmouth%2C_Iowa), where it intersects [Iowa Highway 191](/source/Iowa_Highway_191), then continues east to [Harlan](/source/Harlan%2C_Iowa), where it intersects [U.S. Highway 59](/source/U.S._Highway_59). It continues east from Harlan and intersects [Iowa Highway 173](/source/Iowa_Highway_173) at [Kimballton](/source/Kimballton%2C_Iowa) and [U.S. Highway 71](/source/U.S._Highway_71) at [Hamlin](/source/Hamlin%2C_Iowa). It then continues to [Guthrie Center](/source/Guthrie_Center%2C_Iowa), where it intersects [Iowa Highway 25](/source/Iowa_Highway_25) and [Panora](/source/Panora%2C_Iowa), where it intersects [Iowa Highway 4](/source/Iowa_Highway_4). It then intersects [U.S. Highway 169](/source/U.S._Highway_169) three miles (5 km) before entering [Dallas Center](/source/Dallas_Center%2C_Iowa) and ends at a freeway interchange with [Iowa Highway 141](/source/Iowa_Highway_141) in [Grimes](/source/Grimes%2C_Iowa) in the [Des Moines metropolitan area](/source/Des_Moines_metropolitan_area).

Iowa 44 was created on January 1, 1969, when the [Iowa State Highway Commission](/source/Iowa_State_Highway_Commission) reorganized the state's primary highway system.
