His father was James Neilson, an Ironmaster and second cousin of [Walter Montgomerie Neilson](/source/Walter_Montgomerie_Neilson) and his mother was Jane Thomson, daughter of [George Thomson](/source/George_Thomson_(Shipbuilder)), the famous Glasgow shipbuilder. He attended [Merchiston Castle School](/source/Merchiston_Castle_School).

He was the brother of [Willie](/source/Willie_Neilson), [Gordon](/source/Gordon_Neilson) and [Robert Neilson](/source/Robert_Neilson) who were also capped for Scotland.<ref name=Bath1/> In 1891, he made his debut, along with his brother William in the game against {{nrut|Wales}} – it is the only time that brothers have debuted together for Scotland, with the exception of the joint debut of [Gavin](/source/Gavin_Hastings) and [Scott Hastings](/source/Scott_Hastings_(rugby_union)).<ref name=Bath2>Bath, p97</ref> One of the four Neilson brothers played in each of the twenty five matches between Willie and George's debut in 1891, until 1899, when Robert had to withdraw from the [Calcutta Cup](/source/Calcutta_Cup) line-up after breaking his nose.<ref name=Bath2/>
