The river forms at the confluence of [Cholame Creek](/source/Cholame_Creek_(Estrella_River)),<ref name=Cholame>{{cite gnis|240576|Cholame Creek}}</ref> from the north, and [San Juan Creek](/source/San_Juan_Creek_(Estrella_River)),<ref name=SanJuanCreek>{{cite gnis|273493|San Juan Creek}}</ref> from the south, near the town of [Shandon](/source/Shandon%2C_California). From there it flows west-northwest to its confluence with the [Salinas River](/source/Salinas_River_(California)), of which it is a tributary, 8 miles (12.8 km) north of [Paso Robles](/source/Paso_Robles).  Cholame Creek has its headwaters on the southwest side of [Middle Mountain](/source/Middle_Mountain_(Monterey_County%2C_California)) and its tributary, [Little Cholame Creek](/source/Little_Cholame_Creek), begins on the northeast side.<ref>{{GNIS|244906|Little Cholame Creek}}</ref><ref>{{GNIS|245870|Middle Mountain}}</ref>  The creek drains the [Cholame Valley](/source/Cholame_Valley), which is bordered by [Diablo Range](/source/Diablo_Range) on the east and [Cholame Hills](/source/Cholame_Hills), a northern extension of the [Temblor Range](/source/Temblor_Range), on the west.<ref name=Cholame/> The average [precipitation](/source/precipitation_(meteorology)) in the area ranges from {{convert|11|to|17|in}}, increasing northward.

A portion of the Temblor Range drains into San Juan Creek, but the nearby [Carrizo Plain](/source/Carrizo_Plain) has been cut off from the [watershed](/source/drainage_basin) by [tectonic](/source/tectonic) action. The creek also drains part of the [La Panza Range](/source/La_Panza_Range) in the southwest of the watershed. The vast majority of the land in the watershed is rangeland, but there is also some cropland, pastureland and developed areas.<ref name=SanJuanCreek/>
